- Registration (Complete Online) or (Download Form)
- Summer Schedule (Complete Online)
The following items must be completed, signed and brought to the first appointment with your therapist:
- Cancellation Policy (Complete Online) or (Download Form)
- Consent to Evaluate/Treat
- Feeding and Swallowing Profile (if applicable)
- Food Diary (if applicable)
- PRESCRIPTION from your primary care physician (with Diagnosis Code) for speech therapy
- Sensory Checklist (if applicable)
- Credit Card Authorization (Complete Online) or (Download Form)
